New Patients

You're in great hands

Our awesome front desk is here to assist in your patient experience; from scheduling, insurance verification and billing, and getting you set up with the right therapist for your goals, they’re here to help ensure you feel welcome, safe, and taken care of.

What to expect

Expect one of two formats for your visit:

Initial Evaluation - 60 minutes with your Physical Therapist

  • Show up 10-15 minutes early to check in and ensure all necessary paperwork is complete so that you begin and end your appointment in a timely manner.

  • Wear comfortable clothes that will allow your therapist to access your injured area(s).

  • For your first visit, you will spend 60 minutes with your physical therapist to discuss your injury, history and goals. This will include a movement assessment, education on your condition, treatment if appropriate, and exercises to support your recovery.

Follow Up Visit - 30 minutes with your Physical Therapist

  • Feel free to arrive early to check in, warm up with your exercises or use our equipment as instructed by your physical therapist.

  • You will spend 30 minutes, one-on-one with your physical therapist, engaging in a blend of manual therapy and corrective exercises as needed to reach your goals.

  • If appropriate, your treatment may continue independently/lightly supervised another 10-30 minutes if you and your physical therapist determine you would benefit from additional exercises following your treatment session.

What to bring to your Initial Evaluation

  • If we are billing your insurance, please bring your doctor’s referral, your insurance card(s) and photo ID.

  • If you had surgery, please bring a copy of your operative report - can be requested from surgeon’s office.

  • Any relevant diagnostic imaging: X-Rays, MRI, CT scan, etc.

  • Comfortable clothes that will allow your therapist to access your injured area(s).

  • Please arrive 15 minutes early to allow time for your paperwork to be processed and reviewed by your physical therapist prior to your appointment time.

  • Most importantly, a can-do attitude!

New Patient Forms

Depending on the area of your injury, please fill out one of the following form packets prior to your Initial Evaluation:
